Effects of Client-Centered Occupational Therapy Employing the COPM on Quality of Life, Depressive and Psychiatric Symptoms in a Client With Chronic Schizophrenia: A Case Study

Abstract
Objective : The aim of this study was to investigate the effects of client-centered occupational therapy employing the Canadian Occupational Performance Measure(COPM) on quality of life, psychiatric symptoms, and depressive symptoms of a client with chronic schizophrenia.

Methods : The client was a 54-years-old male diagnosed with paranoid schizophrenia who was an inpatient. The goals were set up through the COPM that was used to identify self-perceived performance problems. The client-centered occupational therapy was conducted for four weeks. This study compared posttest to pretest scores on the COPM and evaluated quality of life, psychiatric symptoms, and depressive symptoms through the World Health Organization Quality of Life Scale Brief Version(WHOQOL-BREF), the Brief Psychiatric Rating Scale-Expanded(BPRS-E), and the Korean Version of Calgary Depression Scale for Schizophrenia(K-CDSS) respectively.

Results : After the intervention, performance score increased by 5.34 and satisfaction score increased by 4.34 on the COPM, score of quality of life increased from 60 to 92, score of psychiatric symptoms decreased from 68 to 43, score of depressive symptoms decreased from 19 to 7.

Conclusion : This study demonstrated client-centered occupational therapy employing the COPM is effective to improve quality of life and reduce depressive and psychiatric symptoms by eliciting interest and facilitating active participation of the client with schizophrenia. Therefore, client-centered occupational therapy which applies individualized goal and program was suggested to be utilized more widely.
